YOUNG PERSON TRANSPORT PERMISSION FORM
If your young person requires a lift TO AN INY FRIDAY NIGHT, HOME AFTER AN INY FRIDAY NIGHT or TRANSPORT TO OR FROM AN INY CG OR ONE OFF EVENT - a parent or guardian needs to complete the following permission form.
This form only needs to be completed once in the calendar year.
The form below is a permission form for parents to fill out on behalf of their young person.
The form gives permission for a young person to be transported to or from youth by an authorised Youth Leader or authorised Parent.
Who will transport your young person?
Authorised Youth Leaders or authorised Parents have completed an induction process to ensure they follow our Safer Churches & Safe Transport Guidelines.
People we authorise will have the following requirements:
Authorised Youth Leaders: P-Plater or higher license with a fully registered and comprehensively insured vehicle who have completed our Safer Churches volunteer training.
Authorised Parents: Fully licensed driver with a fully registered and comprehensively insured vehicle who have completed our Safer Churches volunteer training.
Have a Working With Children’s Check (WWCC) number.
Active member of Imagine Nations Church.
Communicate clearly with a parent or guardian of the young person you are dropping home with pick up times & drop off times.
We will do our best to provide transport for your young person, however, there may be some weeks where an authorised driver is not available and therefore we are not able to promise to provide transport on a weekly basis.
